In today’s AI-augmented world, we often hear warnings
about AI hallucinations, instances where models generate incorrect or
fabricated information. But there’s a quieter, less-discussed risk emerging;
human errors that AI systems mistakenly trust, reinforce, and scale.
This phenomenon, Human-in-the-Loop Bias, occurs when
AI systems assume human feedback is correct by default. The result is a subtle
but powerful feedback loop where AI over-trusts humans, humans over-trust AI,
and small mistakes become systemic failures. Human-in-the-loop (HITL) design is
widely adopted to improve AI safety and performance. It’s meant to ensure that
humans, experienced, rational, and context-aware, correct AI as needed.
But what happens when the human is tired, rushed,
misinformed, biased, or simply guessing? AI systems often take human
corrections as ground truth. If those corrections are flawed, the AI “learns”
